Learning Preference Survey

School Year 2020-2021

DOE public schools offer blended/in-person learning  (a mix of in-person days at school and remote-learning days at home) and fully remote learning. Learn more about blended/in-person learning and the 2020-2021 school year on our Return to School 2020 web pages.

Changing Preferences
  • At any time, you can use this form to choose fully remote learning for your child.
  • At this time, you may not request to switch to blended learning (a mix of in-person days at school and remote-learning days at home)
  • Your child’s school will keep you informed of its specific learning model and your child's schedule. If you have questions, please reach out to the school directly.
If you have any difficulty completing this form online, call 311.

Consent Form to Participate in Random COVID-19 Testing

Please note that students in blended learning must submit a consent form to participate in random testing for COVID-19. The test is quick, easy, and painless, and it helps protect our school and broader communities--learn more  on our website.
You can submit your consent for testing one of two ways:
  • Through your New York City Schools Account (NYCSA) under ‘Manage Account’, or
  • By completing and signing the consent form available here.
